Cavity: Most likely there is tooth decay in the vicinity that weakened the tooth, or a filling that left the tooth structure thin. See a dentist.

See below: Were there fillings in the molars? Any pain or swelling around them? May have had tooth decay or hairline fracture in tooth. See dentist for thorough exam. S/he can then better tell u what happened & why. Do you get adequate calcium & vitamin d, magnesium, & phosphorus in your diet? They're needed for strong teeth.
