My 3 month old has been breastfeeding only and i introduced a soy formula bottle today and now she is vomiting , what can that mean ?

Allergic/intolerant : The simple answer would be that baby is intolerant to the foreign protein or other component of the feed.This could be due to an allergy to one of the components or just a digestive rejection. It might also be co-incidental & due to a stomach virus or oher agent but i think that's unlikely.
