He needs to see Dr: Could have a torn cartilage or other internal derangement. Needs to be examined and treated correctly.

Needs evaluation: Knee swelling in a 19 year old. Unless there has been trauma to the knee a torn cartilage would be very unlikely. Have him see a rheumatologist.
