Unknown: Gramma's wisdom sez will lead to arthritis, but so will wear & tear / use & abuse for 50-100 years. As it is usually a fluid-shifting noise or ligament/tendon sound, it should not interfere with the joint function, although there may be some mild joint enlargement over time.

Tendon snapping: Most clicking sounds around the ankle are the peroneal tendons snapping over the peroneal tubercle. While it is harmless it can lead to tenosynovitis and or degradation of the tendon. If painful physical therapy can help. Surgery is not necessary if not worsening.
