Not sure of your q?: If you are wondering if your head could still hurt from the damage done during your assault 20 years ago, i would say "could be" but would need to examine you and know more about it. If the pains have been constant since then, i would see someone but not worry. If they are changing or growing worse, I am recommending getting seen much sooner.

Traumatic cephalgia: This may be a refractory case of post-concussion head syndrome. A mixed headache syndrome is likley. Need an MRI to rule out chronic subdural hematoma, vascular malformation, meningioma, etc. Consult a neurologist.
