Rarely: Chronic constipation (obstipation) can cause dilation of bowel, rarely can cause an ulcer in bowel, rarely can cause perforation, which could lead to death. Some conditions that cause severe constipation might be severe enough (if untreated) to cause death.

Rare: Having said that. In the very young and very old severe constipation can be very serious and needs to be recognized and treated accordingly.
