Epididymitis: Exam shows a red, tender, swollen lump on the affected side of the scrotum. Tenderness is in a small area of the testicle where the epididymis is. There may be enlarged lymph nodes in the groin area, and a penile discharge. A rectal examination = enlarged or tender prostate. Ultrasound exam, cbc, urinalysis, and tests for chlamydia and other potential causes are all helpful - but exam is key.
