Can heartburn be felt in the back or is it always in the chest?

Yes: Heart burn is most commonly felt in the front stomach and/or epigastric area (middle of chest), and sometines in the throat if accompanied with reflux. The back however is another common place where heart burn can oocur. An ulcer in the posterior aspect of your intestine or stomach for example could be felt as back pain.
