Medications best: Pcos can be treated by a surgery called an ovarian drilling. It is easier, cheaper and safer to treat it with medications first. Surgery should be a last resort.

Possible: A common treatment for pcos in the past was surgical treatment by wedge resection of the ovaries. Most women did resume normal cycle after that surgery - at least for 4 to 6 months. That is not a common treatment now - fertility rugs are more often used.
