Either option: Your patellar tendinitis should not worsen with either position at night- pick the one that is most comfortable for you. Would recommend regular stretching and icing of the patellar tendon on a daily basis. Talk to your orthopaedic surgeon to understand your best treatment decisions. Working with physical therapist can be very helpful.
