Proper brushing: Use a soft toothbrush, make circular motions holding the tooth brush at a 45 degree angle to your teeth, with the tips of the bristles between your teeth and the edge of your gums. Spend 2 minutes brushing and flossing every time after you eat, but at least twice a day, and especially before you go to sleep. Demonstrate how you brush and floss to your dentist to ensure you're doing it right.

Ultrasonic: The most effective way to brush is with an ultrasonic toothbrush (sonicare, oral-b, rotodent), because they can reach a lot more than you can with a manual. Twice a day for an average person is good, morning and night. However, if you build up tartar quickly, three times would be advised. Ada research shows that flossing is more important than brushing for health.
