Was the nail: Cultured? If it's not a fungus, that could explain the lack of success. The single best treatment is oral lamisil (terbinafine). I don't know what "triamcenolon" is. Triamcinolone? If so, this is a steroid and won't do a thing to treat fungus. Find a podiatrist or derm who knows how to treat nail fungus and go on lamisil (terbinafine), now proven much safer than first thought. As about pulse therapy. Don't treat w/o a cultur.

Laser with pulsed: Dosing of Lamisil (terbinafine) tablets. Assuming of course you have the correct diagnosis of fungus. It is very important to eliminate fungus from your shoes with ultraviolet light ie. Sterishoe or at least an anti fungal spray. Lamisil (terbinafine) alone is ok too or laser, but the combination can be effective and pulsing 7 days on 21 days off repeating 2 times. You may not need blood tests with pulse dosing of lamisil (terbinafine).
