Definitely: See your doctor who treated previous fistula. May have only partially healed.

Yes: Absolutely. This happens. As a fissure is healing, it can form a small subcutaneous sinus that becomes a fistula. I have seen that several times. It heals very well because it usually involves no sphincter muscle and is readily treated with a simple fistulotomy.
