VP shunts: The reservoir is there to provide an overflow mechanism among other things. If drainage is delayed into the peritoneal cavity for whatever reason, then the bulb will fill and feel hard, and when empty will be soft. If there is a persistent issue discuss with the neurosurgeon who inserted this.
