Imaging study?: I imagine that if you have been evaluated extensively, you have had a head ct or an mri. If these are normal, i would take this as reassuring information. Migraines come in all sorts of flavors! some do last only a couple of hours, but can be very severe with light sensitivity, vomiting, even visual symptoms. There are excellent medications to treat migraines now. Do talk with your doctor.

Good question: Migraines and what causes them are somewhat of a mystery. Some people get migraines from clenching/grinding their teeth, often without knowing they are doing it. You may want to try a dental orthotic like an nti (see nti-tss.Com) if you haven't already. Just a suggestion.
