Ankle Injury: This type of injury rarely requires surgery. This is what happens when you "sprain" your ankle. Mri is extremely sensitive so don't fret with the results. More important is what your physical exam is like and then appropriate treatment including supervised physical therapy is critically important to reduce the overall disability time.

Not usually: If the space between the fibula and tibia is disrupted, the bones further apart, then surgery is necessary to restore the ankle joint. The anterior talo-fibular ligament is often torn in able sprains and can repair itself.

No you shoulde not: You have a mild high ankle sprain and a low/regular sprain. The term "sprain" does mean a tear of a ligament. It should be treated with ice, rest and physical therapy. I would only suggest surgery if you were to have multiple recurrent injuries in the future that continue after appropriate care.
