Very complicated: Depends on the cause- if the leaky valve is due to the low ejection fraction or if the low ejection fraction is due to the leaky valve. Both can result in enlarged heart/heart failure. Medications such as ace inhibitors and beta blockers can be very helpful. If due to blocked heart arteries, angioplasty may help. Bottom line is knowing what is the cause of the low ef- then you can best treat it.

Depends: Depends on the cause of your heart failure. In any case, you should be on a betablocker (such as Carvedilol or metoprolol), an ace inhibitor ( such as lisinopril, enalopril) and spironolactone (if you tolerate it). These are proven medications that help strengthen the heart muscle. If you meet criteria, a biventricular icd placement may also help.
