Many causes of pain: Many things can cause big toe. Without a known injury, possibilities could include a painful arthritis, fractures, or infections. I would recommend evaluation by a podiatrist.

Gout?: If this came on out of the blue and is very painful, red and swollen it is suspicious for gout. Infection is a possibility as well. Gout responds well to any non steroidal anti-inflammatory. Ibuprofen 600-800 mg three times per day should produce relief in a day or so.
