Gout: Classically gout can attack the big toe. If it's swollen, red and exquisitely tender it could be gout, best verified by an exam and a blood test for uric acid. Assuming you haven't banged it, it might also be just a simple arthritis. Regardless, both often respond to anti-inflammatory meds like Aspirin or ibuprofen.

Needs to be examined: Pain could be as a result of many different things. One would need to do a thorough history and physical exam and possibly some laboratory and radiology exams to determine the exact cause. Causes of pain may include: trauma (fracture, tendon or ligamentous tear, arthritis) infection metabolic conditions (gout) musculoskeletal or biomechanical abnormality. Neurologic conditions (neuropathy).
