Thoughts...: I'd want to know if the urine was clear looking or the urine culture clear. I would also look for other causes of your bladder pressure and back pain. Constipation can cause the symptoms you describe. Kidney infections can present with fever, one sided flank pain, painful urination and a positive urine culture, so what you're describing seems less likely to be that, but still possible.
