See physician: This patient needs to be seen by his or her physician to review all medications, mental status changes, and any illnesses that may be contributing to these changes. Only then can appropriate medication decisions be made.

Depends: Delirium due to alcohol or drug withdrawal is treated differently than delirium (acute brain failure). If due to "acute brain failure", we look for modifiable precipitants to address while also understanding the innate vulnerabilities to delirium that a person may have. Pain meds are not always the precipitating culprit but are frequently scapegoated.
