What are the accountability of the primary care physician and specialists/nurses to me? how can you have any occupation which take money from me and don't have to be accountable?

Profession: It seems as if you are entering a relationship with an antagonistic approach which truly puts a bad taste in either party's mouth. Professionals choose their careers out of a sincere desire to help. I understand your goal to seek a responsible party but you should also factor in : how accountable are you for your health? Doctor generally provide advice & counseling to the best of their abilities.

Confusing question: The attending physicians/specialists & nurses are responsible for providing each patient with care based on standards of need & care established within their local community. Should a dispute arise you have opportunity to question any involved with that care.On the other hand, you pay for the opportunity to receive the care available, not a guarantee that it will meet your expectations.
