Very uncommon: Adrenaline production by adrenals is very reliable in response to stress and is almost never lost. Diabetics with advanced autonomic neuropathy don't make adrenaline when they get hypoglycemic, but they do in response to other stimuli. When adrenals are removed surgically, we don't replace adrenaline (just cortisol), and people do fine.
