Hernia: If your pain occurs only when you are straining to lift weights , defecate, sneezing or bending, then you may have an inguinal herniation. If it is constant, you may have epididymitis, a torsion or a neoplasm. So as not to lose your testicle, cause bowel ischaemia , or mess up your sex life, go see a physician.

Inguinal hernia: Pain that extends down into your testicles can be any number things including an inguinal hernia, epididymitis, varicocele, hydrocele (the last two are less likely). You'll have to be examined by a physician, preferably a surgeon, to make a diagnosis.
