Hidden source: There may be a hidden focus of infection which is seeding or you may have a weakened immune or cell response system. Hygiene is also important, as is ruling out diabetes.

Wider infection: The "bugs" that got to you in the first place may have found a bit of a home to keep reseeding. Have you been treated with a goof tissue penetrating antibiotic?
