Vasospasm: Nicotine is a vasoconstrictor (causes blood vessel constriction or spasm) which can cause a heart attack. In addition, chewing tobacco causes local cancerous lesions in the mouth, cheek, tongue and throat. The stress of the fight against the cancer can cause a heart attack.

Nicotine: Chewing tobacco can affect the heart similarly as cigarettes. It also can cause oral cancer.
