Urticaria forms raised weals, yes ? mine are perfectly flat. they appear at most twice a day and last for about 2-3 hours, on legs and hands.

Yes, usually: Urticaria or hives can present in interesting ways. Typically the skin lesion are raised red, well circumscribed, and very itchy. The size of the wheal (raised part) may be minimal to a few mm or widespread over most of the body. Itching is even variable--some minimal and some very severe. Most will wax and wane over hours; some (more serious ones) may persist for days.
