Fibroids/endometrios: Several reasons for dyspareunia: uterine fibroids, pelvic endometriosis, pelvic adhesive disease, anatomical variants such as retroverted/retroflexed uterus, mullerian anomalies, psychological stress/depression, chronic vaginitis (yeast) or interstitial cystitis. Please see a gynecologist to evaluate this further. Laparoscopy is certainly one option after appropriate evaluation.

dyspareunia: Dyspareunia can have many different causes. In your case deep thrusting causes pain. In some patients deep thrusting may lead to the tip of the penis hitting the cervix and many women find this uncomfortable. If you doctor sees no anatomic abnormality simply avoid positions in which deep painful thrusts is a problem. Hope this helps.
