Difficult to say.: The diagnosis of pcos is often made when a patient has irregular periods, signs of increased androgens (acne, hair growth on face, chin, abdomen), and many cysts on the ovaries. These cysts are usually there at the time of diagnosis. Keep in mind that the ultrasound helps in diagnosis of pcos, and we don't usually follow the cysts unless there is pain, etc.
