4 or more days/week: I recommend a minimum of 4 days a week of exercise (however, it does not have to be all in the gym.). I recommend this number because once this is achieved, you will have exercised more days than you have not. Additionally, there should be some days of for rest and recovery.

It depends: It depends on your level of fitness, general health, and exercise intensity. 2 or 3 times a week would be a minimum. If young or in good shape exercise every day with a few days of rest would be ideal.
