Bug type different : Oral thrush is caused by a fungal overgrowth while herpes is caused by a virus. Site of lesions also help visually to distinguish, but a biopsy is needed to confirm.

Fungus vs Virus: Thrush( monaliasis) is a fungus. Herpes is a virus, it shows as pinpoint swellings that burst into ulcers. Thrush sows as whitish plaques or hairy tongue.
