Most do: I cannot answer for all dermatologists, but we do in our office, and i believe that most of us do. It is probably wise to call the office before making an appointment and ask.

Varies by practice: Some do, some don't. Often the ones that charge for a consult will deduct that fee from the cost of the procedure. Our consults are complimentary.
