Absolutely: Lungs usually don't hurt (they don't have nerve endings), but the wrap around them, called pleura, can hurt. Throat and esophagus pain can definitely cause referred pain to the back.

Yes: Pain in the chest can be referred into the neck and back. Nerves can travel fr the front of the chest to the back as well (known as intercostals).
