See your doctor: Pain that severe needs to be worked up by your doctor in person. He/she will listen to you, perform an examination and possibly order some tests to see what's wrong with you.

Spinal fluid leak: An accidental puncture of the dura (membrane around nerves) can result in a leak of fluid that persists when standing, but is relieved by lying down. If lasting more than a couple days, then a simple blood patch (drawing a couple cc of your blood, and reinjecting to epidural site to plug it) would almost immediately resolve. If at home, try caffeine + any other pain reliever you use for headache.
