One possibility: If the "water baloon" happens closer to the shoulder joint it may represent a acromioclavicular joint (ac) cyst, a fluid-filled sac arising in the setting of a long-standing full-thickness rotator cuff tear. Subsequent mechanical wear of the articular capsule of the ac joint, allows passage of fluid from the glenohumeral joint into and often beyond the ac joint. Did you do an MRI of the shoulder?
