Possibilities: Any activity that involves heavy lifting, or increasing your intraabdominal pressure does refer that pressure into the pelvis. It could be a pulled or mild sprain of an internal muscle or groin pull, it could be dilated vessels/ structures in the scrotum (like a varicocele), but it may be a small hernia also. They can be hard to diagnose especially if it is small.

Obturator hernia: This could be a hernia if your obituary or canal, a space in your pelvis. This is hard to diagnose on physical examination and may require special imaging such as ct scan.
