Work up: You will need radiological studies, nerve conduction studies. Usually the treatment is physical therapy but sometimes surgery is needed.

Evaluation: You should be seen by someone with an interest in treating thoracic outlet syndrome. This can be a surgeon, neurologist, orthopedic surgeon, etc. Physical therapy is almost always the first line of therapy.

Choices: Would advise working with an experienced physical therapist, and have found terrific outcomes with Feldenkries approach. About 85% respond to therapy and home exercise. Failing that, 15% might be surgical candidates, but would certainly give at least 6 months focusing on conservative approaches first.
