Surgery or nothing: It is called a varicocle and unless symptomatic requires notherapy. If symptomatic need surgery.

Varicocele: It sounds like a Varicocele. You need to consult a urologist but they can either perform surgery or an interventional radiologist can embolize it via a very small incision.
