My friend is 15 and every so often he coughs up quite a bit of blood, what does this mean ?

Have him see MD!!!!: While there are certainly benign things that can cause this such as an inflammed tonsil, there are enough less benign things that can cause this that he absolutely needs to be seen by a doctor. A doctor can examine the throat, order a chest x-ray to check the lungs and any relevant blood tests. Help him schedule appointment if need be!
