Many things...: Could be that you're not brushing properly (even though you are brushing). Could be diet (no amount of brushing will overcome someone who sips on mt. Dew throughout the day). Could also be that you need more Fluoride exposure (to help strengthen the teeth). Dry mouth can also contribute to the problem as well. I would recommend having a consultation with your dentist to determine the cause.

Bacteria: Most dental problems involve bacteria (decay process & periodontal disease- gum & bone recession) if you cannot control the bacteria, then you will continue to have problems. Other dental problems may be caused by traumatic injury, genetic pre-disposition, poor quality prior dental care, infrequent dental care, poor diet, lack of sleep, daily stress, bad habits, smoking, poor general health etc.

More than brushing: Need to have regular dental appointments, proper brushing (need to do so correctly and after eating, if possible,) and need to floss! Some people are just prone to having problems even with immaculate home care. You may need more than 2 visits to the dentist per year.
