Surgery: Male breast reduction surgery is very common and can take on a variety of forms. Sometimes simple liposuction can remove the unwanted tissue but doesn't work well in men that have a lot of extra breast skin or firm tissue just deep to the nipple/areolar skin. In such situations skin removal needs to be considered and can be done with a "peri-areolar" approach in some patients.

Probably surgery: Even in spite of massive weight loss, gynecomastia usually persists. You may have a problem of too much skin, in addition to breast tissue present. Many young men have a mild case of gynecomastia, with only puffy nipples. Others have a more serious case. Most require surgery for correction, even mild puffy nipples.
