Discuss with surgeon: It does increase your chances of a blood clot. Speak to your surgeon. You shouldn't smoke and shouldn't take antiinflammatory medications.

Probably: If your surgery is anticipated to involve a prolonged period of bedrest or inactivity, you would be at increased risk of developing blood clots if you were to start or restart the pill. You should discuss this with your spine surgeon.
