Not sure of ?: Pleural effusion is not a diagnosis, it is a symptom of some other underlying disease. Pleural effusions can be successfully drained, but depending on what is causing them (heart failure, pneumonia, cancer, tuberculosis and many others) they frequently come back. With a long smoking history, i would be concerned about cancer, and the pleural fluid should have been tested. He must stop smoking.

Question: Not sure exactly what is your question. I agree with dr. Wartenberg. Longterm smoking increases risk for lung disease both cancer and non-cancer. Effusions have many causes. Father needs a good primary care physician. You may find the following link useful in ref to cancer: http://www.Cancer.Net/cancer-types/lung-cancer.
