Not by doctor: Processes vary, but drivers' licenses are never taken away by doctors -- only by designated authorities in each state. Also it depends on the degree & progression of symptoms. If concerned about a patient's attention span, concentration, reaction time, or memory for directions, etc, legally the doctor must report this to the state's dmv or equivalent. Talking w/patient & family would occur first.

It depends: Some people with mild alzheimer's disease may be still able to drive safely. It also depends on the state regulations that may require notification of the department of transportation if somebody gets the diagnosis.
