They shouldnt hurt: Generally speaking, hemorrhoids should "hurt." they can obviously bleed and cause itching and discomfort but if there is pain when you have a bowel movement it may be a fissure instead. You also wont "get" a hemorrhoid all of the sudden with straining but you may burst a hemorrhoid and it could bleed. Hemorrhoids may get bigger with straining, but everyone has some degree of internal hemorrhoids.

Internal hemorrhoids are swollen and make the passing of the stool difficult need to strain hard and have to use enemas to have a bm. The stool is flat but soft, i take a stool softener every evening.
