Im in my early 20's, male, have patchy hair loss and a painful lump that has formed in my left breast. what could this be?

Several things: Hair looks like alopecia areata which is an autoimmune condition. It is important to get blood work done to rule out thyroid, anemia problems. See a dermatologist for treatment which could be topical steroid as well as injection steroid. Breast needs to be seen by pcp asap.
