Let me explain: Femoral offset is the distance from the center of rotation of the femoral head to a line dissecting the long axis of the femur. In case of total replacement hip the the offset is considered as the distance from the center of rotation of the femoral head to a line bissecting the long axis of the stem. Normal femoral offset varies between 30 amd 60 mm.

Malalignment of hip : Most likely refers to a malalignment of the femur(thigh bone) in the hip socket. It can cause prssure in the joint capsule and nerves that surround the area.
