History and exam: Costochondritis would usually be associated with persistent tenderness if the ribs at the costchondral junction; gallbladder problems cause right upper quadrant pain worse after a fatty meal; reflux causes usually an anterior chest pain worse after meals and with laying flat. There are also tests such such as an upper GI endoscopy that can give a lot more information as well.
