Directly: Please be upfront and direct. Tell her that second hand smoke can cause heart disease, lung cancer, asthma, and predispose the children to ear infections, sinus infections and multiple allergies.

Smoke in kids: Causally mention something you recently heard about smoking, 2nd hand smoke, and kids; and use that as the starting point.
