Yes: One may have .Coronary artery spasm or so called prinzemetal angina.Abnormal vascular reactivity of the micro circulation can induce angina.Cocaine also can induce coronary vasoconstriction and heart attack!

Yes: While people with high blood pressure, high cholesterol, and/or overweight have a higher risk of heart disease, not having these factors does not mean that one does not have heart disease. The risk of having heart disease is lower than if those factors were present, but some degree of risk will still be present.
